Request A Second Tag

This form is for Capitol Mall Parking tag holders to apply for an additional tag. You may possess no more than two parking tags, and may use only one at any one time.

Red tag holders will be charged the monthly fee for $5 for the second tag. White assigned tag holders will be charged a one-time fee of $5 for a second tag.

When your application is received and confirmed, you will be mailed an additional tag via Statehouse mail.

Note: You are not required to get two tags to drive multiple vehicles to work - you may instead register multiple vehicles on one tag, and just move the tag into whichever vehicle you drive each day. There is no additional charge for this.

By submitting this application, you authorize your departmental payroll clerk to deduct an additional $5.00 from your pay each month for an additional standard (red) parking tag, or a one-time fee of $5.00 for an additional assigned (white) parking tag. Additional monthly deductions for an additional standard tag will continue each month thereafter until (a) termination of your employment, or (b) written notice by you of cancellation of one or both tags, to Facilities Services, whom you authorize to notify your payroll clerk of the action. You also acknowledge your understanding that no interest is to be paid by your employer on accumulated funds.
